Adult Case Management:
HOM provides adult case management to adults meeting specific eligibility criteria, conduct intake assessments, coordinate full comprehensive clinical assessments, link them to appropriate resources in the community, ensure their basic needs are met, assess their strengths and needs, develop individual treatment plans, monitor for progress, and make changes as needed to ensure adequate progress is being made All services are provided on an individual basis taking into consideration that all individual are unique and require one on one planning.
