We follow a five-step model for Children’s Case Management:

  1. Referral & Intake – Accepting referrals from families, providers, or schools, followed by an intake meeting and consent coordination.

  4. Implementation – Coordinating appointments, arranging transportation, and ensuring timely service delivery.

  5. Ongoing Monitoring – Conducting quarterly reviews, tracking outcomes, and planning for transition or discharge when appropriate.
